Luxe Home Detox LLC is dedicated to safeguarding your Protected Health Information (PHI) in compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). By signing this consent, you authorize Luxe Home Detox LLC to use and disclose your PHI for:

  • Administering treatment.

  • Day-to-day operations of the practice.

  • Medical record keeping stored in a HIPAA-compliant Electronic Medical Record (EMR) system.

Permitted Uses and Disclosures: Luxe Home Detox LLC may use and disclose PHI without authorization in the following circumstances:

  • For treatment, payment, and healthcare operations.

  • If a client requires hospitalization due to illness or injury.

  • If someone poses a potential threat to themselves or others.

  • In cases of abuse, neglect, or domestic violence, as required by state or federal agencies.

  • To persons involved in your care, such as family members or close friends, with your consent.

Your Rights: You have the right to:

  • Inspect, copy, amend, or remove your PHI.

  • File a complaint if you believe your privacy rights have been violated. Complaints can be submitted to Luxe Home Detox LLC or to the Secretary of Health and Human Services at:
